Lifestyle Health is a partnership between the allied health services workingat the Division. The program is designed to improve the coordination of a Team Care Arrangement that will support GPs manage patients with a chronic condition. The service is located at the Division office, which has the advantageof referring the patient to the one location.

How Does it Work?

Services will be delivered individually or in a group format*. Theprograms will apply motivational interviewing principles to encourage positive lifestyle modificationsand provide physical activity and nutrition advice that supports the clients health needs.

* Note: Group format NOT applicable for Dietitian or Exercise Physiologist under a Team Care Arrangement (Item 723)

How do I Refer?

Please download a copy of the Referral Form, complete the required information and fax to the Division on 9585 2144. A staff member will contact the patient to organise group or individual appointment.
