Advanced Care Directive Association: Demetia

 

On 30th July the Advance Care Directive Association (Inc) launched 'A Plan of Care -a book to help people make health and personal care decisions for a person in NSW who has dementia"

The book aims to assist family carers to consider and develop a plan (in partnership with medical and other health/service providers) to clarify aims of care, once decision-making ability for healthcare has been lost by the patient. The book is based on work done locally in recent years with families of patients with advanced dementia.

Regional Medical Advisory Committee (RMAC)
 

The St George Division of General Practice is a fund holder for the Department of Health & Ageing in relation toRACF/GPinterface. One of the requirements for Accreditation of Nursing Homes is to have an active Medication / Medical Advisory Committee. With the large number of RACFs in the Area it became apparent thatconsolidation of the many individual facility committees, into a regional committee, would be the most efficient and beneficial to all concerned.

The committee has formulated Terms of Reference and meets bi-monthly. Agendas and minutes of each meeting are circulated to all key stakeholders. Membershipis comprised of a number ofvisiting GPs, a geriatrician, supply pharmacists, accredited pharmacist, Directors of Nursing, hostel managers and nursing representatives from St George Hospital.
