Background

 

A Federally funded Program to enhance integration between the Acute and Primary  CareSectors.The ABHI Co-ordinator works 2 days per week from the St George Hospital and 2 Days per week at the Division office. This role provides GPs and Practice Staff a link to them and all Hospital services. This position may be regarded as a trouble shooter one. 

Any issues pertaining to these areas such as communication, discharge processes andreferral pathways may be directed to Sandy via mobile or fax (see resources). Sandy will work through any issue to a satisfactory conclusion for all parties. This position includes an active role in many key committees and working parties.It gives General Practice in the St George area a voice.

 

Projects

 

- Yellow Envelope: Developed to improve information transfer between Aged Care Facilities and St George Hospital. Implemented December 2008 

- Peri-natal Mental Health Pathway: Development of pathways in conjunction with Mental Health Program Officer to link perinatal clients with appropriate counselling services following referral from perinatal mental health committee

- Community Nurse and Practice Nurse partnerships: Developed partnerships in collaboration with the practice nurse program officer to enable practice nurses and community nurses to better manage ambulant patients with minor wound issues

- Calvary, Sydney ACAT discharge: Working closely with teams at this site to support the implementation of anelectronicallybased discharge summary and admission notification.

- Electronic Discharge from St George Hospital:Negotiations, resource development, ARGUS involvement and clarification for GPs
